KVK Application and Information Request Form
GENERAL EXPLANATIONS
Personal data owners defined as the relevant person in the Personal Data Protection Law No. 6698 (“PDP”) (hereinafter referred to as “Applicants”) are granted the right to make certain requests regarding the processing of their personal data in Article 11 of the PDP Law.
In accordance with Article 13/I of the PDP Law, applications to be made to our Company, which is the data controller, regarding these rights must be submitted to us in writing or through other methods determined by the Personal Data Protection Board (“Board”).
It is important that the personal data we keep about you is accurate and up-to-date. Therefore, please notify us if there is a change in your personal data. In accordance with the PDP Law and relevant legislation, you;
– You can learn whether personal data is processed,
– You can request details regarding the information we collect,
– You can learn for what purpose this information is collected and how it is used, and the third parties to whom personal data is transferred domestically or abroad, or
– In case of incomplete or incorrect processing, you can request that it be corrected, updated or deleted if the reasons requiring processing are eliminated, including notifications to be made to third parties to whom it is transferred.
In this context, applications to be made to our Company in “written” form can be forwarded to us by printing out this form and writing “Personal Data Protection Law Information Request” on the envelope or in the subject;
• By the applicant’s personal application,
• By notary or registered mail,
• By sending it to the electronic mail (KEP) address registered to our Company’s address.

(Data Controller) in accordance with your rights as a Data Subject under the Personal Data Protection Law No. 6698 (“KVK”).
